Moxley Public Health sets themselves apart in many ways. One way is the personal and “boutique” style services that our clients receive when navigating the journey to complete their Community Health Needs Assessment (CHNA) and Implementation Strategy (or CHIP).

With our full-service consulting package, we facilitate the whole process and pretty much do it all for you! Moxley Public Health will collect all the data (both primary and secondary data), conduct the interviews, develop and send out the surveys to the community, and write the reports. We will work together as a team to customize your needs, problem-solve, and we will walk you through making decisions to select priority health needs, choose strategies and action plans to address those health needs, and encourage collaboration with community partners.

What we do

COMMUNITY HEALTH NEEDS ASSESSMENTS (CHA OR CHNA)

We gather and assess health needs data for a community or organization that provides a “snapshot” of assets/resources and priorities for improvement. This is done through partner collaboration, community data collection (such as focus groups, interviews, and surveys), and use of existing data. In the end, we create reports that are visually appealing, IRS and PHAB compliant, useable, and allow hospitals, health departments, and other organizations to make decisions that improve community health.

IMPLEMENTATION STRATEGIES/ IMPROVEMENT PLANS (CHIP)

We create an actionable plan to address prioritized public health problems based on the needs assessment data. Our improvement plans focus on health equity by identifying health disparities in the community during the needs assessment process and then using strategies that target priority populations. The plans are developed collaboratively with community partners, are PHAB and IRS compliant, and include a work plan to ensure our clients are successful in their implementation.

TECHNICAL ASSISTANCE ON WORK PLANS AND EVALUATION PLANS

We work with clients on implementing and developing their work plan and evaluation plan to ensure that they successfully implement their strategies to address health needs. This includes identifying who is accountable for each strategy, creating a timeline with annual activities and SMART objectives, and choosing indicators to monitor and evaluate progress.
